We proudly support the aged and disability community, focusing on providing the highest quality care to help individuals live healthier, happier, and more independent lives.

With mobile teams across Australia, we are able to assist the elderly, people living with a disability, and those requiring in-home rehabilitation, offering essential community care services directly to their homes. If in-person visits are not feasible, we also offer telehealth programs to ensure you continue to receive the support you need.

Our approach is centred around providing community home care services that are tailored to your unique needs. We aim for regular visits from the same team member to foster a strong connection and understanding, ensuring that your care is personal and effective. We are committed to providing a client-centred approach, with an emphasis on clear, ongoing communication between all people involved in your care.

All Plena Home Care visits occur at your private residence, retirement home or residential aged care facility

Rehabilitation after a fall, injury, accident or illness

Management of acute and chronic pain

Falls risk assessment and prevention

Strengthening and flexibility exercises to maintain physical abilities such as walking and getting up from a chair

Improvement of general deconditioning or physical decline due to long hospital stays and prevention of hospital readmission

Incontinence management

Home safety assessments and home modifications

Assessment, education and prescription of adaptive equipment (E.g. mobility aids, shower chairs, toilet frames, pick up sticks)

Cognitive assessments and retraining

Speech Pathology
Our experienced speech pathologists are here to support your swallowing, speech, voice, language and/or communication needs. When conducting swallowing assessments, we observe how you normally eat and drink, provide practical strategies, swallowing exercises and advice on how to prepare food and fluid modifications to ensure you are eating and drinking safely. We listen to your concerns, ask questions, conduct tests and make clinical observations and recommendations tailored to your individual needs.
