Why we transformed our service delivery to Physiotherapy and Occupational Therapy Packages
Our Clinical Wellness Packages focus on client-driven goals: staying at home, reducing hospitalisations, and improving health outcomes in line with Support at Home reforms. We shift the focus from reacting to incidents to reablement – conducting early risk assessments, identifying potential health issues, and implementing proactive wellness strategies. This holistic service extension also enhances access to care and empowers clients while supporting their independence.
