Plena Healthcare offers a variety of services within residential aged care facilities, supporting providers to make allied health services available to residents who need them.

The Plena Care Pathway is our unique service model supporting consumers throughout their entire residential aged care journey. We also offer services to support providers and their staff, including assessment and care planning, equipment review and prescription, training and wellness, and collaboration, communication and consultation.

Physiotherapy
Our Physiotherapists are highly experienced in managing a wide range of acute and chronic conditions which affect movement, function, and independence.
Occupational Therapy
Our Occupational Therapists work with clients to determine what is most important to them, including a review of self-care, environmental assessments and physical rehabilitation
Podiatry
Our Podiatrists conduct a neurovascular and dermatological assessment for each new client. Based on these results they will provide clinical recommendations individualised for the client’s foot care needs. Our podiatrists can also conduct footwear assessment or make custom foot orthotics on request.
Dietetics
Our Dietitians are experts at creating personalised nutrition programs to address specific dietary needs. We provide recommendations that combine best practice nutrition, preferences and experiences.
Speech Pathology
Our Speech Pathologists support with swallowing, speech, voice, language and/or communication needs. We listen to concerns, ask questions, conduct tests and make clinical recommendations tailored to the individual needs of each person.

Plena Care Pathway

The Plena Care Pathway is a sustainable model that supports providers to ensure residents have access to allied health care via an efficient and agile service model.

Improved restorative outcomes delivered by the Plena Care Pathway can reduce the cost of care as well as reducing risk of falls and pressure injuries. It supports providers in maximising improvements in function under AN-ACC Classification, is aligned directly to the person-centered care approach of the quality standards and improves key NQIP outcomes such as reductions in falls and user satisfaction measures that directly influence star ratings.

Indicators that influence aged care star ratings including:

  • Activities of daily living (ADL) – Percentage of care recipients whose ADL function has declined.
  • Hospitalisations – Percentage of care recipients who presented to hospital.
  • Consumer experience – Percentage of care recipients who report good or excellent experience of the service.
  • Quality of life – Percentage of care recipients who report good or excellent quality of life.

Our therapists, managers and dedicated client liaison team work in partnership with referring partners, providers, clients and their families to ensure that the healthcare services being provided meet the needs of the individual.

We believe we have the best Physiotherapists, Occupational Therapists, Podiatrists, Dietitians and Speech Pathologists in the country – all backed by our award-winning employee training. We aim for regular visits from the same team member to facilitate connection and personal understanding, and our client liaisons ensure the therapist’s skills and interests are well matched to the needs of the client.
